Student Teacher Evaluation Forms

Instructions for using the following files:  Click on the appropriate cell in the tables using your mouse.  Once the cursor is in the correct cell, you may type in the information that you are prompted for.  You may also use the arrow keys on your keyboard to select the cells.  If you choose to add comments, please click on the text box below the label "Comments:"  Note, using the arrow keys to get to the text box will not work.  After completing the form, please print it and sign the bottom.
