Proof of Immunizations and Completion of Health Status Report

PROOF OF IMMUNIZATIONS & COMPLETION OF HEALTH STATUS REPORT

Policy:

All students enrolled in the Nursing Major shall have a health status report and demonstrate proof of immunizations prior to the start of the first semester in the Nursing Major. All returning students shall provide proof of updated immunizations to the office of Nursing Student Services at the College of Nursing by August 1 (fall semester) and December 1 (spring semester). 

Purpose of Policy:

     1. Define which immunizations are necessary for admittance into the Nursing Major. 
     2. Define the responsibilities of the College of Nursing and Student Health Services with regards to validation of immunizations and health status report. 
     3. Identify the central location within the College of Nursing and Nursing Student Services where current immunizations and health status reports are located and verified. 

Procedure:

1. All students admitted into the Nursing Major will provide information from a current physical examination by a licensed health care provider and complete all required immunizations. These forms must be on file one month before classes begin. The College of Nursing will provide forms to the student. The forms are: 
     a. Health Status Report
     b. Student Immunization and Testing Form.

2. Every semester, each continuing student will provide updated information verifying current cardiopulmonary resuscitation testing, and any needed immunizations or boosters, as appropriate, to the office of Nursing Student Services.  Annual tuberculin skin testing must be submitted each semester before the annual renewal date has expired to the office of Nursing Student Services. This information is to be submitted on the Student Immunization and Testing Form along with a copy of the student’s CPR testing card.

3. The completed forms are to be submitted to the office of Nursing Student Services one month before classes start.  For all students entering or continuing in the major in the fall, the submission date is August 1.  For all students entering the Nursing major in the spring the submission date is December 1.

4. If forms are incomplete (Health Status Report or the Immunization and Testing Record), letters will be sent to each student from Nursing Student Services indicating that the student will not be allowed to start classes or attend any clinical orientation until all forms are complete and on file in Nursing Student Services.

5. A list of students and their status for Tuberculosis, chest X-ray, Immunization status, Health Status Report, and CPR is sent to each Semester Coordinator, from the Nursing Student Services Secretary, within one week prior to the start of classes.

6. See attached forms:

     a.   Student Health Status Report; and
     b.  Immunizations and Testing Record for the most up-to-date requirements 

7.     Special note: No student will be allowed to attend any clinical experience if they have a positive tuberculin skin test and have not had a negative chest x-ray.  If the student has a positive chest x-ray, the student must be under the care of a health care provider and receive written clearance by the health care provider that they are able to care for patients.  If the student refuses to comply, administrative withdrawal from the College of Nursing will occur.

Administrative Withdrawal: A letter will be sent to the student and the Records Office, from the Dean of the College of Nursing, indicating the date which compliance must occur.  The student will be given reasonable time to comply.  If no compliance occurs by the specified date, the student will automatically be withdrawn from the College of Nursing and the University.  The student must process the withdrawal through the Records Office to be withdrawn for without failure.  Grades transcripted are based on the date of application for withdrawal.
