Application for Admission to the Nursing Major: Accelerated Option

I, request admission to the Accelerated Nursing Option in the College of Nursing at South Dakota State University. I am applying for a position for Fall Semester 2008 in Sioux Falls, South Dakota.

Include evidence of a TOFEL Score for students with English as a Second Language. This score must be 600 or above for consideration for admission into the Nursing Accelerated Option.

REQUIREMENTS:
  • A minimum of a "C" grade in all prerequisite and concurrent nursing required support courses. 
    Please note: 
    1.  Students who have failed 2 or more of the pre-requisit science courses (Chem 106, 108; Biol 221, 231, 325), repeated and passed them on the second attempt will not be admitted to the Accelerated Program. 
    2.  Students who have failed one pre-requisit course (Che 106, 108; Biol 221, 231, 325; Psyc 101, Soc 100, 150, 240, 250; HDFS 210; NFS 321; and Stat 281 or HSc 445), repeated and failed the same course a second time will not be admitted to the Accelerated Program. 
    3.  Students who have been enrolled in a previous nursing program, and have failed a course, will not be admitted to the Accelerated Program. 
    4.  Students who have taken Anatomy or Physiology more than seven years prior to their admission date, will be required to update these courses.
  • No pre-requisite may be taken more than twice without written approval from the Dean of the College of Nursing.
  • Any exception to these policies must be approved in writing by the Dean of the College.
  • A minimum cumulative GPA of 2.8 for all post secondary course work and a minimum cumulative grade point average of 3.0 on a 4-Point Scale for Nursing pre-requisites (Chem 106, 108; Psyc 101; Soc 100, 150, 240, 250; Biol 221, 231, 325; HDFS 210; NFS 321; and Stat 281 or HSc 445).
  • Transfer Courses:  All courses from other institutions should be transferred to SDSU as soon as possible. If a course is still in progress, please attach a current transcript that shows midterm grades for the course(s) in progress from that institution.
  • Transfer Students:  The application form to SDSU must be submitted prior to application to the major.
  • Complete the Nursing Major application form, include your essay, and submit by the due date. All application materials are due in the office by the due date not postmarked by the due date.
  • Submit 2 letters of professional reference from an employer or instructor by the due date.  The subjects to include are:  e.g. accountability, professionalism, time management skills and personality traits that make the candidate an excellent choice for an accelerated nursing program and health care professional.
  • Students with English as a Second Language need to meet specified requirements. Call Nursing Student Services for information 605-688-4106 or surf to the online Nursing Student Handbook and read the information regarding English as a Second Language. English as a Second Language is defined as a student who was instructed and spoke primarily in a language other than English in the K-12 grades or primary and secondary schooling.
  • Students who meet the requirements will be contacted for a personal interview.
DIRECTIONS FOR COMPLETION OF THE APPLICATION:

I. Read the application carefully.

II. Section I : List the grade(s), number of credits, and school where you received the grade(s) for each of the following General Education courses and required pre-requisite nursing courses you are currently taking or have completed. (If you have taken a course more than once report all grades for the course.)
For courses you are currently taking, put a (*) on the grade line. Complete the credit line blank and indicate where you are enrolled in the course.

III. Complete Section II, noting example of how to compute Grade Point Average.
 
IV. Respond to the questions in Section III.
 
V. Section IV: Complete the essay, submit with the application.
 
VI. Submit all application materials to:  SDSU Nursing, Sullivan Health Science Center #260, 2300 North Career Avenue, Sioux Falls, SD 57107.

FIGURING GPA EXAMPLE

>The grade point average (GPA) is obtained by dividing the total grade points by the number of hours completed.

SECTION IV:  SELF EVALUATION ESSAY
Respond to the following self-evaluation questions in essay format on a separate paper.  Responses must be typed, double spaced and limited to 300 words per question.  Please place your name on the upper right hand corner of the first page, along with the date.

1. Describe talents, aptitudes, or gifts that you possess and how these could contribute to a nursing career. (For example: art, music, computer literacy, like people or good communicator).  You must include the following:

a. Demonstrate self-understanding and acceptance beyond routine reporting of actions, facts, and experience.
b. Present creative application of your talents/gifts in relation to nursing.
c. Discuss how nursing benefits from the application of art as well as science.
d. Demonstrate an understanding about nursing as a career choice.
2. Describe any life experiences that have contributed to your interest in nursing as a career.  (For example: leadership roles or community service).   You must include the following:
a. Self-reflection on life experiences.
b. Demonstrate how your life experiences can contribute to your nursing practice.
c. Discuss your motivation and commitment to nursing.
d. Articulate your personal desires and strengths.
3. Describe how you envision yourself, as a nurse, practicing in the health care system in the next five years. 
You must include the following:
a. How your practice may be affected by health care issues in the United States.
b. Reflect on your beliefs about nursing’s place in the U.S. Health Care System.
c. Present your personal career goals.
d. Discuss commitment and motivation in relation to a professional career.
