CPR, Health Status Report and Immunization Requirements

CPR, HEALTH,  STATUS REPORT  AND IMMUNIZATION REQUIREMENTS

Policy Title: CPR Certification 
All nursing students are to maintain current cardiopulmonary resuscitation certification (CPR). Certification can be either:

(1) American Heart Association (BLS Healthcare Provider Course C),

(2) American Red Cross (Professional Rescuer), or

(3) Proof of CPR (1 & 2 Man, Infant Resuscitation, and Choking).

A.  Purpose of Policy

1.  Document proper CPR certification on a timely and ongoing basis.
2.  Define the responsibility of the College of Nursing with regards to validation of CPR certification.
3.  Identify the central location within the College of Nursing where CPR certification documents can be located and verified.

B.  Procedure

1.  All students in the Nursing Major must provide proof of current CPR certification to the office of Nursing Student Services in the College of Nursing.

a.  A copy of the certification card with expiration date must be submitted two weeks prior to the start of classes each semester for all returning students.
b.  Nursing students admitted into the first semester of the Nursing Major must submit a copy of the CPR certification card with expiration date prior to Semester One Orientation.

2.  If students have not complied with submission of a copy of the CPR card, returning students will not be allowed to attend clinical orientation or clinical and new students will not be admitted into the Nursing Major.

3.  Notification of deficiencies will be sent to each student from the office of Nursing Student Services indicating that the student will not be allowed to attend any clinical orientation or clinical within one week prior to the start of classes for new students and two weeks prior to the end of the semester (for returning students, not including Final's week). A copy will be sent to the Semester/Term Coordinators.

4.  A copy of the most current CPR card will be kept in the student's file and will be included in the student database.

CPR
Students must maintain a current CPR card. In order to contract agencies’ requirements, the student must complete a course which includes “one person and two person” rescue and care of adults, children, infants, and choking resuscitation. Acceptable courses include: American Heart Association Basic Life Support “C”, American Red Cross Professional Provider Care, or evidence of having completed the equivalent training.  A copy of each card will be maintained on file in the office of Nursing Student Services.

Health Status Report
A Health Status Report must be completed and submitted to the office of Nursing Student Services prior to the first clinical care assignment. If the Semester/Term Coordinator does not have verification of a completed Health Status Report by the time of the first clinical experience, the student will not be permitted to enter the clinical area. The form is given to the student at the time of admission to the nursing major. Additional forms are available from the office of Nursing Student Services. The completed Health Status Report will be maintained in the office of Nursing Student Services until graduation. RN Upward Mobility students will complete the Health Status Report as a part of the College of Nursing application process.

Immunizations
Students are required to have a completed Immunization and Testing Record  filed with the office of Nursing Student Services. This form must be completed and filed before the student may start clinical. The form is provided to the student at the time of orientation to the Nursing Major. Additional forms are available from the office of Nursing Student Services. RN Upward Mobility students will complete the form as part of the College of Nursing application process. It is the responsibility of the student to provide the College of Nursing with documentation of annual required immunization, completed hepatitis series, and booster immunization.

Verification of Documents
All record of CPR, health status, and immunizations are centralized and recorded in the database in the office of Nursing Student Services. A copy of the current data is available from the office of Nursing Student Services. Data will be entered as forms and cards arrive. Semester/Term Coordinators will have access to records of the student's current status. It is the responsibility of the office of Nursing Student Services to monitor student compliance prior to clinical assignments and to communicate that information to the Semester/Term Coordinators. At the time of graduation, all records will be placed in the College of Nursing Dean’s file.
