CPR, HEALTH, STATUS REPORT AND
IMMUNIZATION REQUIREMENTS
CPR
Students must maintain a current CPR card. In order to contract agencies’
requirements, the student must complete a course which includes “one person and
two person” rescue and care of adults, children, infants, and choking
resuscitation. Acceptable courses include: American Heart Association Basic Life
Support “C”, American Red Cross Professional Provider Care, or evidence of
having completed the equivalent training. A copy of each card will be
maintained on file in the office of Nursing Student Services.
Health Status Report
A Health Status Report must be completed and
submitted to the office of Nursing Student Services prior to the first clinical
care assignment. If the Semester/Term Coordinator does not have verification of
a completed Health Status Report by the time
of the first clinical experience, the student will not be permitted to enter the
clinical area. The form is given to the student at the time of admission to the
nursing major. Additional forms are available from
the office of Nursing Student Services. The completed
Health Status Report will be maintained in the office of Nursing
Student Services until graduation. RN Upward
Mobility students will complete the
Health Status Report as a part of the
College of Nursing application process.
Immunizations
Students are required to have a completed Immunization
and Testing Record filed with the office of Nursing
Student Services. This form must be completed and filed before the
student may start clinical. The form is provided to the student at the time of
orientation to the Nursing Major. Additional forms are available from the office
of Nursing Student Services. RN Upward Mobility students will complete the form
as part of the College of Nursing application process. It is the responsibility
of the student to provide the College of Nursing with documentation of annual

Verification of Documents
All record of CPR, health status, and immunizations are centralized and recorded
in the database in the office of Nursing Student Services. A copy of the current

access to records of the student's current
status. It is the responsibility of the office of Nursing Student Services to
monitor student compliance prior to clinical assignments and to communicate that
